Background

Chronic pain is complex and will always be difficult to manage. In the 1990’s there was an over optimism that opioids were a useful therapeutic tool to manage chronic pain. Gradually the same emerged for gabapentinoids. Over the following decade it became clear that this optimism was misplaced and that in fact they were causing more harm than good. The USA especially experienced a crisis culminating in the President declaring a public health emergency. In the UK a similar pattern emerged, but thanks to better control of the supply of medicines the level of deaths has not occurred. A series of media campaigns have highlighted the risks and lack of efficacy of strong opioids. Nice have highlighted good practice in the prescribing of opioids and gabapentinoids. The term de-prescribing has now been applied to the reduction of opioids and gabapentinoids.

Patients tend to fall into three groups when de-prescribing. Those that intend to taper but are highly motivated, those that are open to tapering, but need sustained support and those who are highly addicted, on unsafe amounts of prescribed drugs and may be highly manipulative. Differing approaches are needed with GP’s as the prescribers required to deliver these.

Effective systems and individual understanding are needed.

Proposal

This workshop will cover potential approaches to de-prescribing. Experienced clinicians will talk through these and address any gaps in knowledge using a variety of learning approaches. Following this table-top discussions lead by a facilitator will support you to come up approaches that will work for you and your practice.
