Three options for patients to provide their feedback
Paper and Pen
Paper responses can be printed out and given to patients by receptionists.
Once completed, the receptionist (or any appropriate member of staff) can add the answers to the system themselves.
Online web link
A unique web link will be provided for patients to use at home and for paper entries to be manually added by practice staff.
Each practice is be given a code for their feedback survey on www.leavemyfeedback.com

Automatic reporting
Every month our system will send you an automatic report ready to pass on to NHS England.
