NORTH RIDING COMMUNITY HEALTH NETWORK PCN are excited to offer a new opportunity for an enthusiastic, forward thinking clinical pharmacist to become involved in the development of the clinical pharmacist role within our practice network.

The post holder will be passionate for their role to be integrated into primary care will work across all four member practices Ampleforth and Hovingham, Ayton and Snainton, Derwent and Sherburn and Rillington. GP mentorship will be provided by the GP’s within the practice and continued professional development is supported by all.

The duties of this role includes providing medication reviews including complex structured medication reviews for patients and answering queries regarding medications to patients and practice staff. Using their expert knowledge on medicines the post holder will provide clinical leadership on medicines optimisation, quality improvement and prescribing safety to the clinical teams within our PCN. The post holder will expand communications with community and hospital pharmacy to improve access to healthcare, assist with workload and improve patient outcomes.

Our PCN is located on the edge of the North York Moors National Park and our surrounding area includes the Howardian Hills an area of outstanding national beauty, white sandy beaches of the Yorkshire Coast and picturesque views of the Wolds not forgetting the market town of Malton, Yorkshire’s famously renowned foodie destination. This is a picturesque tranquil area within easy reach of York and excellent road and rail links to major cities. The area offers affordable housing and an abundance of outdoor activities.

Our PCN has a combined list size of 39,000 patients, each member practice has a CQC rating of good in all areas and 3 out of the 4 practices are dispensing practices.
