To provide high quality and effective administration support within the West Mendip Symphony Hub team. The aim of the service is to work closely with other Health and Social Care organisations to co-ordinate and provide seamless care, wrapped around the patient, their carers and the patient's support network while reducing the risk of hospital admission.
