An exciting opportunity has arisen for General Practitioners to join our highly motivated, personable and patient focused Complex Care Clinical team to deliver the best care to our patients in Yeovil and the area to the West of Yeovil.

We would welcome enquiries or applications from existing and newly trained General Practitioners, and are flexible on the number of sessions.  The salary is being reviewed to ensure it remains competitive and attractive.

Detailed job description and main responsibilities

To manage patients who have the highest health complexity, which often encompasses multiple co morbidities, frequent hospital admissions, psychosocial issues, frailty and polypharmacy. The aim of the role is to improve continuity of care across the different health interfaces – primary, secondary 

To work alongside the primary care team to manage high complex patients in the community.

Provide medical leadership and clinical support to the multi-disciplinary team.

Attend Huddles within GP practices to discuss and offer advice/review identified.

Attend secondary care meetings to help manage patients identified by these teams, to optimise their care.

Support discharges of high complexity patients back to community and liaising with their primary care teams.

Attend hospital or community MDT meetings, safe guarding meetings and Best Interest meetings.

Manage high complexity patients with long term conditions in the community and developing disease specific care plans and treatment escalation plans where indicated.

Involvement in the development of the Complex Care service.

Conduct face to face consultations with patients requiring GP input.

Prescribe in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.

Provide extended appointments and/or home visits for patients where this need has been identified.

Attend regular practice team ‘huddles’ to help manage patients identified by the practice team as requiring further medical input.

Record clinical information in the appropriate paper or computerised clinical software systems.

Manage your own time, delivering on objectives and being accountable for your own professional actions.

Participate in the significant event and complaint processes relating to complex care cases.

Adhere to the professional standards as set out by the GMC.

Maintain an up to date caseload list.

As the complex care model continues to evolve in order to offer best patient care, so the job description may be amended to reflect these changes. As a result the complex care GP needs to be flexible and adaptable in order to respond to any changes that occur.
