Clinical Pharmacist – Willington Surgery

The Clinical Pharmacist will have a key role in supporting delivery of the new Network Contract DES Service specifications. For the new Structured Medications Review and Optimisation requirements this will include tackling over-medication of patients, including inappropriate use of antibiotics, withdrawing medicines no longer needed through NHS-led programmes such as low priority prescribing and medicines optimisation more widely. For Enhanced Health in Care Homes residents will benefit from regular clinical pharmacy led medicines reviews.

Willington Surgery Overview

We have a team of 27 with a clinical team made up of 5 GP partners who work between 7 & 5 sessions per week, 1 salaried GP, 3 Practice nurses, HCA and Phlebotomist. We pride ourselves on being well led by all and we have an open inclusive culture. We are situated in a modern, purpose-built premises with 12 consulting rooms and cover Willington, Repton, Finden and other surrounding areas, with a total capitation around 9500.

We offer excellent services to our patients and encourage all feedback; we aim to be constantly open to improvement, and frequently celebrate the praise we receive from our patients. The team work hard to improve processes within the practice to benefit patients and promote a positive work environment for all.

Our PCN is made up of 8 practices in total covering the south of the city of Derby.

The postholder will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a varied, and sometimes challenging environment is essential.

The successful candidate will have excellent interpersonal and communication skills, and be organised, patient and empathetic. They will have experience of working in health, social care or other support roles including direct contact with people, families or carers.

Core Duties:

The Clinical Pharmacist will : 

  • Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
  • Uphold a prescribing qualification, or training to become a prescriber, and will work with and alongside the multi-disciplinary team across the Primary Care Network.
  • Engage in the CPPE course if not already completed.
  • Take responsibility for the care management of patients with chronic diseases with the GP and Nursing teams and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple long-term conditions (in particular COPD and asthma) and people with learning disabilities or autism (through STOMP - Stop Over Medication Programme), chronic kidney disease, atrial fibrillation, ischaemic heart disease etc.
  • Provide specialist expertise in the use of medicines while helping to address both the public health and social care needs of patients in the network.
  • Provide leadership on person centered medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, while contributing to the quality and outcomes framework and enhanced services.
  • Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care.
  • Support the practice with repeat prescribing including working with medicine orderline processes and electronic repeat prescribing.
  • Support further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
  • Develop relationships and work closely with other pharmacists across networks and the wider health system.
  • Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties) and anticoagulation.

Other Duties:

  • Work within the policies of scheme and Practices.
  • Maintain a good working knowledge of health and safety procedures.
  • Promote client involvement in the management of the service.
  • Participate in regular supervision sessions and appraisals with your line manager. Attend training and development activities as identified and participate in meetings as required.
  • Maintain administration systems and workspace used in a clean and tidy condition.
  • Maintain a good working knowledge of Health and Safety procedures and fire precautions and operate the correct procedures and participate in policy development and data collection where appropriate.
  • Work flexibly to meet the needs of clients.
  • To undertake any other duties appropriate to the grade and purpose of the job as may be agreed by the post holder.
