Hours of work: 37.5 hours per week

Monday - Friday 9am – 5pm

Salary: £10.46

Main Purpose of Job

We have an exciting, new and developing opportunity for a Care Co-ordinator to join our Primary Care Network, supporting our 3 GP practices across the West Suffolk area to assist in re-shaping Primary Care across our communities.

The post holder will be a key member of the General Practice team, building relationships with other community providers, community services and working closely in partnership with other staff within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service

The Care Coordinator’s role will provide support with coordinating all key activity including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred. The post holder will provide general administrative and clerical support to staff as directed; undertaking a variety of tasks to assist with the day-to-day running of the Health Centre.

Care-Coordinator Responsibilities

Undertake work in line with PCN directed priorities.

To work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients.

To be the direct liaison for multi agencies, patients, carers and care homes, supporting with the management and coordination of care.

To develop excellent working relationships with all the GPs and wider service networks including the voluntary sector, other GP practices, adult social care and hospitals.

To provide co-ordination and navigation with the aid of digital tools for people and their carers across health and care services.

To work closely and in partnership with the Social Prescribing Link Workers and Health and Wellbeing Coach(es), referring patients that have been identified as potentially benefiting from these services.

To utilise population health intelligence and reporting to identify work with high-risk patient populations and specific cohorts; such as those with long term conditions and learning disabilities.

To document and monitor aspects of patient co-ordination and to support data collection and audit using the patient administration system, Emis.

To raise awareness of health promotion and NHS health checks in practices.

To provide administrative support for the COVID-19 vaccination programme, arranging clinics and inviting patients.

To support National NHS Screening and Immunisation programmes and coordinate clinics and recalls.

To support Quality and Outcome Frameworks (QOF) and other DES/LES specifications.

To attend meetings, providing administrative support when required.

To provide administrative support to the Practices Patient Participation Group (PPG), attending meetings and taking meeting minutes.

To coordinate, monitor and help deliver project and services/ campaigns.

To provide business support to the Practice Manager, PCN manager and Administration Manager.

To undertake any other administrative / clerical task as reasonably delegated to by doctors or management.

Excellent inter-personal skills and ability to work within a team-based environment.

Able to work with a great degree of flexibility and adaptability.

Self-motivating and able act on own initiative .

Excellent written and verbal communication skills.

Accurate and methodical.

It is essential applicants meet the following criteria

Proficient in the use of IT; Microsoft office suite packages (including word, excel and powerpoint), internet and email, data operating systems.

Previous experience of database administration.

Consistently undertakes work that is accurate and detail oriented.

Experience of handling confidential information.

Ability to co-ordinate tasks.

Strong organisation and problem solving skills.

Excellent time management skills and have the ability to prioritise requests to deal with conflicting demands and meet deadlines

Proven ability to provide excellent customer service (internal and external).

To be able to collate and disseminate voluminous and complex information.

It is desirable applicants meet the following criteria

Working knowledge of EMIS web
