The Rural Practices PCN have an exciting opportunity to recruit a Health Coach to work within the PCN multidisciplinary team.
This is a PCN funded role and the post is jointly employed by the member practices which include Queen Camel Medical Centre, Wincanton Health Centre, Bruton Surgery, Milborne Port Surgery and Millbrook Surgery. For this appointment Queen Camel Medical Centre will be the host practice providing monthly payroll services and acting as an agent on behalf of the Rural Practice Network. The postholder will predominantly work with patients registered at Queen Camel Medical Centre but may occasionally work across network practices.
Main duties of the job
You will provide support to patients to take pro-active steps to improve the way they manage their physical and mental health conditions based on what matters to them.
You will support people to develop their knowledge, skills and confidence or to build their patient activation – in managing their health and care, to improve their health outcomes and quality of life.
The post is 24 hours per week based at Queen Camel Medical Centre.
As a Health Coach, the post holder will have responsibility for proactively reaching out to, monitoring and coordinating care for a defined cohort of patients in the practice. The post holder will be a key member of the primary care team, playing a critical role in engaging patients and encouraging them to take an active role in their health.
Overview of your organisation
Queen Camel Medical Centre is a successful dispensing practice with a GOOD CQC rating. The practice has 5 GP Partners, 2 salaried GP's and an ANP working alongside the nursing, dispensary and admin teams. The practice consistently receives excellent results from the national patient survey.
