COMMUNITY DIRECTOR FOR EAST CARDIFF CLUSTER ( 2 POSTS)

Salary: £20,195.52 per year (2 sessions per week)

Initial 12 months mutual assessment; then a 3 year rolling contract, extended annually (for a further 3 years) by mutual agreement

We are looking to recruit two new GP Community Directors (Cluster Leads) to join the South and East Cardiff Locality Management Team and take lead of the South East Cardiff Cluster and East Cardiff Cluster

Working within the Primary, Community and Intermediate Care Clinical Board, the Cluster Community Director will undertake a pivotal role in developing the Primary Care Cluster in line with the transformation agenda set by Welsh Government. Working closely with local primary care providers, social care services and hospital and community based services, the Community Director will be influential in the development of new models of care to meet local needs and developing the autonomy of the Cluster.

As part of the role you will be expected to provide clinical advice and leadership on cluster issues. You will need to maximise and maintain the communication network within your cluster practices including assessing and addressing local CPD issues and facilitating the evolution of Cluster Network Development meetings to become mutual business clinical / learning forums. You will also co-ordinate and chair regular cluster CDS sessions as well as lead on the provision of primary care clinical pathway development. You will take a lead role in developing the maturity of the cluster, with specialist support being provided from within the PCIC Clinical Board. You will be supported in your role by the designated Locality Manager to whom you will be operationally responsible, with additional support being provided via a Cluster Development Manager. The post is professionally responsible to the Clinical Board Director, via the Locality Lead Community Director.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
