Job summary

An exciting opportunity has arisen within Diamond Health Group. We are seeking an enthusiastic, pro-active and forward-thinking Nurse Practitioner or Specialist Paramedic to join our skilled multi-disciplinary team and to deliver a high standard of patient care. A prescribing qualification is desirable. The hours for the position are negotiable up to full time.

Main duties of the job

The role predominantly supports the management of acute minor illness and on the day appointments. Working alongside our existing Acute Care team of Nurse and Paramedic Practitioners and with the support of the on-call GP and Clinical Pharmacist, you will:

  • Consult with patients in person and remotely, that present with new onset symptoms
  • Clinically examine, diagnose and implement a treatment plan with GP involvement as appropriate
  • Refer patients to appropriate care settings and signpost
  • Undertake home visits
  • Carry out learning Disability health checks

We are also a training Practice for GP Registrars and Physician Associate students and pride ourselves on the development of our teams, in a supportive environment.

You will be joining a welcoming and highly experienced Clinical team of GP’s, Acute Care clinicians, a Practice Pharmacist, Nurses, HCA’s and Health Coaches; that in conjunction with our participation in the Yeovil Primary Care Network, are able to provide an holistic approach to patient care with access to an MSK service, Care Home team, Health Coaches and Cancer Care Co-ordinator.

Job responsibilities

The post holder is an experienced Nurse Practitioner or Specialist Paramedic who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment including prescribing and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process but with the full support of the wider clinical team in making treatment plans. They will work collaboratively within the general practice team to meet the needs of patients and supporting the delivery of policy and procedures.

Clinical responsibilities:

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
  • Work with patients in order to support compliance with and adherence to prescribed treatments
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
  • Support and manage health needs of women presenting for family planning, sexual health consultations
  • Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health
  • Conduct visits to patients in either their own home or a care home environment
  • Liaise with GPs to admit patients to secondary care if they are in acute medical need
  • Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other relevant care providers 

Person Specification

Experience

Essential

  • A minimum of 3 years paramedic work experience
  • Evidence of appropriate continuing professional development activity
  • Proven ability to evaluate the safety and effectiveness of their own clinical practice

Desirable

  • Experience of working in a Primary Care environment

Knowledge and Skills

Essential

  • Ability to assess and manage patient risk effectively and safely
  • Time management skills and the ability to prioritise workload
  • Well-developed data collection and IT skills
  • Excellent interpersonal, verbal and written communication skills
  • Reflective practitioner
  • Ability to analyse data and information, drawing out implications for the individual patient/impact on care plan
  • Ability to establish and maintain effective communication pathways within the organisation and other external providers
  • Full driving license

Qualifications

Essential

  • Qualified paramedic or specialist paramedic registered with the HCPC or a Nurse Practitioner qualification working at degree level with NMC registration

Desirable

  • Independent non-medical prescriber qualification
  • A detailed understanding of a general practice environment
  • An understanding of QOF framework
