Job Summary

Eyam Surgery is a small, but busy practice in the heart of the Peak District. We have strong nursing team that consists of a Nurse Practitioner, 3 HCAs and Community Matron that have all worked at the surgery for at least 7 years. In June, our Community Matron is going to retire so we are looking for an enthusiastic registered nurse who is just as passionate as our current team, and who wants to make a real difference to the people who live in our area.

Main duties of the job

  • carry out physical examinations and treatments in the patient's home
  • refer patients to a specialist
  • manage the care and support patients receive
  • identify patients who may be at risk of being admitted to hospital when they don't need to be
  • manage services to make sure the focus of care is on the home and community for as long as possible
  • teach patients, carers and relatives to spot changes that could lead to conditions getting worse
  • organise extra support, like home care or respite care
  • Chair the monthly MDT meeting at the surgery, as well as attend the monthly in-house Quest meetings
  • make sure policy guidelines and procedures are followed
  • maintain patient records

Job description

Job responsibilities

  • Maintain patient confidentiality
  • Manage information with sensitivity
  • Work closely with colleagues at the surgery and those who work outside the surgery
  • Work to prevent unnecessary admissions to acute care, when possible, as well as timely discharges by using services in and out of the surgery
  • Use clinical skills and experiences to assess both the physical and psychological needs of the patient
  • Anticipate care needs and develop appropriate management plans, referring to other services as appropriate
  • Facilitate important but often difficult conversations with the patient, carers and or professionals regarding end-of-life planning
  • Complete long term condition reviews, take blood and administer vaccines within the patient's home
  • Carry out written and dynamic risk assessments
  • Chair and cascade information during the surgery's monthly multidisciplinary meeting

Person Specification

Experience

Essential

  • Experience of working within a nursing team
  • Able to carry out physical examinations and comprehensive health assessments
  • Experience of working with older people or those with complex needs
  • Experience of managing patients with long term conditions
  • Experience of managing a case load

Desirable

  • Experience of working in a GP  practice

Qualifications

Essential

  • RN First Level Nurse
  • Current NMC registration
  • First Level relevant degree or equivalent

Desirable

  • Master's level degree
