Job summary

We have an exciting opportunity for a Lead Nurse to join our team at Wensum Valley Medical Practice. We are looking for a dynamic, committed and experienced individual to work collaboratively with the entire practice team, supporting the delivery of care for the practice population, including patients with acute and chronic conditions. The successful applicant will also provide leadership and direction for the nursing team, including Healthcare Assistants and Phlebotomists.

The team at Wensum Valley Medical Practice are friendly and supportive, and we work collaboratively to deliver a high level of care and service for all patients, who currently number around 12,500.

We are friendly and dynamic, and work across three surgeries in Norwich. We have been providing care to the people of Norwich for many years and have a strong focus on patient care. We are passionate about training and supporting our staff.

The successful applicant will be an experienced Nurse Manager, preferably with primary care experience, or someone from a primary care background who is looking to take the next step in their career development. We welcome applications from individuals with solid clinical and management skills who also has a vision for service development and delivery.

Salary to be agreed based on skills and experience.

Main duties of the job

The Lead Practice Nurse has responsibility for providing operational clinical, managerial and professional leadership for the practice nursing team, consisting of Nurses, Healthcare Assistants and Phlebotomists, working closely with the GP’s and Practice Management Team to deliver the Partnership priorities.

Working as part of a multidisciplinary team, the post holder will be accountable for the delivery and standards of care provided to patients ensuring safe, effective, person centred care. The Lead Practice Nurse will support the practice management team in the review and delivery of clinical policies and procedures and will have a high degree of autonomy.

We are a forward thinking, dynamic and friendly Partnership. We have two GP partners, and we aim to build the partnership to at least four partners in the next two years. We have a young multidisciplinary team including salaried GPs, long term locums, ANPs, highly skilled practice nurses, care coordinator, GP assistants, pharmacists, mental health practitioners and social prescribing link workers. The clinical team is supported by an excellent administrative team with experienced managers. Team spirit is strong as is the commitment to great patient care.

We operate from three locations and provide high quality care to around 12,500 patients. We enjoy excellent working relationships with neighbouring practices, working collaboratively as part of the West Norwich Primary Care Network.

Job responsibilities

Key Responsibilities:

  • Lead in the management of the nursing team, including appraisal and setting of personal development plans, performance and absence management
  • To ensure that all staff are treated fairly and consistently in line with employment policies and procedures
  • To provide leadership and direction to the nursing team to enable the Partnership to meet its performance targets, whilst ensuring the care provided is delivered to a high standard
  • To be responsible for the development and implementation of operational plans and strategies, designed to achieve Partnership targets and objectives
  • To ensure that the nursing team adheres to all clinical governance standards, ensuring safe, efficient and effective care in accordance with clinical based evidence, NICE and the NSF
  • To ensure compliance with Health and Safety policies, ensuring safe working practices
  • Ensure that a robust system for clinical supervision and professional governance and revalidation is implemented for all nursing staff
  • To provide a visible, accessible and authoritative presence in clinical settings from whom patients and families can seek assistance, advice and support
  • To act as the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the Practice Management Team.
  • To oversee the provision of chronic disease clinics, delivering patient care as necessary and referring patients to secondary or specialist care as required
  • To maintain accurate clinical records in conjunction with legislation
  • To support the clinical team with safeguarding matters in accordance with local and national policies
  • To understand practice and local policies, and help to draft and implement policy where required
  • To use excellent communication skills to motivate the nursing team, and in all communications with patients consistent with their level of understanding
  • Develop a culture that improves quality, and takes appropriate action when required to ensure that quality is maintained across the nursing team
  • Contribute to a supportive working environment for the benefit of the practice team and patients, which promote both the profession and the Partnership
  • To lead and participate in audits appropriate to the role

 

 

Person Specification

Qualifications

Essential

  • GCSE grade A to C in English and Maths
  • Registered nurse
  • Evidence of continued professional development

Desirable

  • Post graduate diploma or degree (Chronic Disease Management)
  • Experience of using SystemOne
  •  
  • Experience of leading a nursing team
  • Nurse prescriber
