Job overview

Are you passionate about promoting the physical health care of those with mental health conditions……. if so this role could be for you!

This is an exciting opportunity to be part of a brand new service within Somerset.

As part of the NHS Five Year Forward View, NHS England and NHS Improvement have set out a national target for physical health checks for those patients on the GP register who have severe mental illness (SMI).

We are seeking to appoint two qualified Assistant Practitioners in Mendip and South Somerset to work as part of the wider health and social care team to support GP practices to address the mortality gap and reduce health inequities for patients on the SMI register. We also have a fixed term Mat leave cover available in Bridgewater.

You will work closely with Community Mental Health services and our voluntary Sector partner agencies to deliver holistic physical health assessments and support individuals to understand about their health particularly in managing their physical health needs.

You will have a level 5 vocational qualification such as a Foundation Degree or other Level 5 Diploma in Healthcare or be currently working towards this qualification. (If you are working towards this qualification you would be employed as a Band 3 pending successful completion of the course) Experience of working in a health setting is essential.

You will receive formal monthly supervision and an annual appraisal.
