Job Summary
Cannington Health Centre are looking to recruit a registered Paramedic (HCPC). To be responsible for the provision of paramedical services, delivering care within their scope of practice. This is an ideal opportunity to join our well established, friendly team.
Main duties of the job
To assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the PCN or practice team.
Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services and self-care.
About us
Cannington Health Centre is in a village on the west side of Bridgwater. We are a 4 GP partner practice, with 3 nurses and an HCA. Our patient list size is 5,200 and we are a dispensing practice. We work in collaboration with the other local practices and are part of the Bridgwater Bay PCN. Cannington Health Centre has an excellent record of QOF achievement and is rated CQC ‘Good’.
Job responsibilities
Assess, diagnose, plan, implement and evaluate treatments/interventions and provide clinical care/management as required
Triage patients and provide the necessary treatment during home visits
Prioritise health issues and intervene appropriately
Work with patients in order to support compliance with and adherence to prescribed treatments or over-the-counter medicines (within own scope of practice)
Recognise, assess and refer patients presenting with mental health needs
Support patients to adopt health promotion strategies that promote healthy lifestyle and apply principles of self-care
Engage in patient care relating to QOF, ensuring read codes are used effectively
Maintain accurate clinical records in line with extant legislation
Reporting on and filing lab results
