On behalf of the South West Cardiovascular Network, Somerset ICB is looking to recruit to the following vacancy, which is within the Planned Care and Integrated Care Team, and part of the wider Commissioning directorate.

Somerset ICS Primary Care CVD Clinical Champion

Based on sessional pay.

The Role:

The South West Cardiovascular Network has funded a primary care CVD clinical champion for each ICS in the South West. The posts are for 1 day a week until March 2023 (12 months funding) and funded on a sessional basis. We are looking for a clinician who is passionate about cardiovascular disease, preventing ill health, and reducing variation and inequalities.

The post is Somerset wide and will focus on reducing variation and implementing quality improvements in the areas of highest need.

Typically, you will support Somerset ICS to improve the outcomes for people with CVD by:

Detection and management of CVD risks in primary care including:

Annual review and management of patients at high risk of CVD-related deterioration

Use of risk stratification tools and case finding methodologies to detect at-risk people

To improve secondary prevention by early detection and treatment of the PHE and NHSE ABC ambitions: A(trial fibrillation), B(lood pressure) ,C(holesterol) in addition to Heart Failure.

Increasing the use of advice and guidance, electronic referral systems (ERS) and other methods of triaging patients e.g. NT-proBNP prior to referral.

Increasing the uptake of digital technology and diagnostic and self-management technology such as the measurement of blood pressure at home.

Improving the uptake of Cardiac Rehabilitation

Participation in CVDPREVENT audit

You will also be required to attend meetings hosted by the South West Cardiovascular Network.
