Fabulous opportunity to work in a friendly, supportive, caring team on the edge of the Peak District near the Stockport/Derbyshire border. Sett Valley Medical Centre is a perfect sized practice of 10,500 patients with a large, mixed skills team who focus on providing high quality patient-centred care to our community. We are a training practice and a member of the Derbyshire Training Hub who are keen to support you in your training needs and proud of our track record of career development of our staff.

We are growing our team of practice nurses, so looking for enthusiastic, caring, knowledgeable, like-minded souls to help improve the health of our lovely patients. Full or part-time. Experience working in General Practice or the community is preferable, and knowledge of chronic disease management and providing childhood vaccinations desirable but will support any training needs. You must be a qualified nurse registered with the NMC.

Main duties of the job

Being a Practice Nurse will give you the opportunity to have a variation in your working week.

Sett Valley Medical Centre is a semi-rural 3 partner practice caring for 10,500 patients, situated in the High Peak of Derbyshire, on the edge of the Peak District National Park, close to Stockport with excellent road and rail links to Manchester.

We are based in a modern purpose-built surgery, with an outstanding CQC grading, and our ethos is to deliver high-quality medical care through our comprehensive, highly skilled team members.

We have a strong and active patient participation group, and good community health care links. The surgery is paper light and uses EMIS Web.

Job responsibilities

Job Summary:

To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team

Job Responsibilities:

Professional:

  • Assessing problems presented opportunistically by patients, dealing with minor illness.
  • Able to document Consultations according to NMC guidelines
  • Chronic disease management
  • Offer family planning and cervical cytology
  • Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions.
  • Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines.
  • Ensure infection control guidelines are maintained.
  • Able to recognize and manage anaphylaxis according to current UK guidelines.
  • Able to perform Cardio-pulmonary resuscitation according to current UK guidelines.
  • Running well person clinics and health promotion programmes by providing a holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise.
  • To set up and run specialist clinics as appropriate.
  • Follow agreed clinical protocols.
  • Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, safe sex, food hygiene, sun protection.
  • Ensure awareness of statutory and local clinical protection procedures.
  • Ability to obtain and document informed consent (either verbal or written).
  • Ability to monitor and manage maintenance of stock and equipment to include refrigeration and emergency equipment.
  • Confidentially of information gained at work must be preserved at all times.

Managerial:

  • Assist patients to identify their health needs.
  • Manage and organise individual patient consultations.
  • Aware of identification and reporting procedures related to professional standards.
  • To attend staff meetings.
  • Support the Practice clinical governance agenda.
  • Contribute to the assessment of service needs.

Educational:

  • Identify personal development and training needs in conjunction with the manager.
  • To participate in continuing education and maintain a contemporary level of professional knowledge and skills.

Personnel:

  • Maintain good professional relations.
  • Take reasonable care of his/her own safety and that of other persons who may be affected by his/her act or omission.
  • Co-operation with the Practice to ensure all members of the team adheres to statutory regulations/policies and codes of practice and departments safety rules.
  • Maintains a caring environment through the support of colleagues.

  • Using personal security systems within the workplace according to Practice guidelines
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
  • Reporting potential risks identified

Person Specification

Qualifications

Essential

  • * Registered Nurse
  • * Recognised qualification to diploma level or above

Desirable

  • Diploma in chronic disease management - diabetes, asthma, COPD
  • Cervical cytology registered smear taker

Experience

Essential

  • * Experience of working in accordance with the NMC Code of Professional Conduct (2002) and the Scope of Professional practice (1992)
  • * Experience of dealing with the public/patients.
  • * Experience of working as part of a team
  • * Able to prioritise workloads
  • * Able to deal with difficult and confrontational situations
  • * Dealing with emergency situations
  • * Dealing with confidential information
  • * Experience of a busy working environment

Desirable

  • Experience of working in a GP Surgery or for the NHS
