Job summary

Crewkerne Health Centre is looking for an enthusiastic and committed Advanced Nurse Practitioner to join the team on a permanent basis. This post available is up to 37.5 hours, Monday to Friday.

Salary will be £37,523 to £51,465 pro rata, dependent on experience. Hours and working patterns to be discussed at interview but hours can be flexible based on the successful candidate.

Crewkerne Health Centre is a friendly practice in a semi-rural market town location, yet with the support of a network of practices behind you so you can concentrate on patient care. We are also part of Symphony, the largest GP provider in Somerset so benefit from the experience and learning across a network of 20 practices.

Main duties of the job

The post holder will be an experienced Nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of patient care.

The post will holder will be able to demonstrate the ability to make differential diagnosis using decision making and problem solving skills whilst making professional autonomous decisions for which they are accountable. They will possess the ability to work independently and to work collaboratively as part of a multi-disciplinary team with other health care professionals and disciplines, supporting the delivery of policy and procedures. NMC requirements for advanced practice must be met.

Job description

Job responsibilities

The Nurse Practitioner will work within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures. In order to work at this level, NMC requirements for advanced practice must be met.

Person Specification

Experience - Essential

  • Minimum 5 years Post Registration Experience
  • Minimum 2 years recent primary/community nursing experience
  • Nurse led management of minor ailments and injuries
  • Nurse led triage

Experience - Desirable

  • Health Needs Assessment

Qualifications - Essential

  • Registered First Level Nurse
  • MSC or equivalent
  • Relevant nursing/health degree
  • Clinical supervision training and experience

Qualifications - Desirable

  • Independent/supplementary Nurse Prescribing Qualification
  • Mentoring/teaching Qualification
