Clinical Director – Somerset Home Treatment (Hospital at Home)
Working pattern - Up to 10 sessions role (up to full time required) – open to job share or less than full time
Tenure – Initial 2 year then continue by mutual review, open to all senior clinicians within Somerset
Salary – Appropriate to clinician’s current national pay scale (incorporating experience and service)
Reports to – Assoc. Medical Director, Charlie Davis
An exciting role is available to work together across the county to implement our home treatment service in Somerset. This hospital at home will care for and treat 280 people, who would otherwise be in hospital. We are seeking to increase leadership at clinical director level. You will join a triumvirate which consists of a project manager and nursing lead. This group will be responsible for the service as it grows and substantiates its role in admission avoidance and reduction in length of stay. The clinical director will work with other senior clinical leads in frailty, respiratory, cardiology and surgery to design and implement a countywide service based in our 4 localities. Clinical sessions are possible within the role either within the service or in an existing service. The role is open to considering a job share arrangement.
We encourage applications from primary and secondary care medical colleagues and credentialed Advance Clinical Practitioners working at a Nurse Consultant level. The Somerset home treatment service is a multidisciplinary team providing care at home along specialised pathways. The clinical director will nurture these pathways whilst supporting cultural and professional change to working within the community providing hospital home treatment. These pathways integrate into neighbourhood services to provide patients and families with holistic care. The place of Home treatment in our current system is shown below –

This is an exciting opportunity to shape integrated care which could be arranged flexibly and considered as a secondment role. The 4 localities for Home treatment are Taunton and West, Mendip, Bridgwater/Sedgemoor and South Somerset. They give opportunity to work across Somerset from any base including Home working.
