THE JOB
Do you like to work autonomously? Are you looking for a role where you can take the reins and be a leader?
We are seeking a driven, innovative and experienced Clinical Care Co-Ordinator with proven leadership skills, in an autonomous position, to join our Primary Care Network. You will be employed by our Cleveland Practice on behalf of the practices in the Trent Care Network. The role will be based in multiple locations across the PCN area.
WHAT WILL YOU DO
The role of the Clinical Care Co-ordinator is to support both the Trent Care Network practice staff and members of the Neighbourhood Team to identify and support people to reduce the risk of unplanned hospital admissions and to effectively support the delivery of person-centred care to those individuals living in care homes.
To work dedicated hours to focus on proactive case managing people and being the preferred point of contact for the patient and Neighbourhood Team to achieve the following objectives:
- To be a proactive member of the Trent Care Network Team and Neighbourhood Team
- To pro-actively engage with people deemed to be at a high risk of hospital admission
- To proactively engage with people and their support staff to understand reasons for the high-intensity use of urgent or emergency care services, and explore plans which can be put in place to reduce escalation.
- To pro-actively engage with the in-reach teams to reduce the length of stay in hospitals
- To be the key contact for people at the practice
- To take a lead in the MDT facilitating multiple agencies to come together and discuss people who require multi-agency or professional advice and support
- To promote personalisation and support the practice and care home staff to ensure appropriate care plans are in place
WHO ARE WE LOOKING FOR?
To be considered for this role you will require the following:
- Registered Nurse Level 1 Postgraduate study in health-related studies relevant to long-term conditions or equivalent experience
- Passed training requirements as outlined by the Personalised Care Institute
- Evidence of continuing professional development
- Experience in care coordination or clinical administration
Desirable qualifications/skills:
- NVQ Level 3 or equivalent
- Qualification/experience within a business administration field Post registration teaching qualification or willingness to undertake
- Experience in a health or social care profession
- Evidence of knowledge of Primary Care Networks and General Practice including an understanding of the GP Five Year Forward View
You will work 37.5 hours per week.

Trent Care Network (TCN) consists of 5 GP surgeries in Saxilby, Gainsborough and Hibaldstow. We cover a population size of 40,198 patients with a range of diversity.
