We are looking for a General Practitioner with a strong interest in working with people with frailty and who aspires to deliver responsive, patient-centred, effective care. The post holder will lead a clinical team for the multidisciplinary home visiting service on a day-to-day basis. They will provide senior clinical triage as well as clinical support and mentorship and undertake home visits where necessary.
The role will involve between four and six sessions over Tuesdays, Wednesdays and Thursdays.
Whilst taking on a lead role in a developing the service you will work alongside other clinicians to develop and implement clinical pathways and meet the unique challenges of our house bound and care home patients. With an ability to assess and challenge traditional ways of working you will be the key to proactively developing others and continually improve the service we offer.
The hours for this post can be flexible with the number of sessions per week negotiable although full days are preferred.
Main duties of the job
The aims of the Team Up service is to support people to live well and independently at home for longer by delivering proactive and responsive care. We aim to reduce inappropriate admissions to hospital and to support patients who are leaving hospital to return and recover at home.
The post holder will provide clinical leadership to a multidisciplinary team of highly skilled and dedicated practitioners including ACPs, paramedics and care coordinators. To support this you will lead regular MDTs to discuss complex patients and to debrief others in the team as required. It will also be a requirement to work well with other agencies including social care, mental health, therapy and district nursing to promote continuity of care. You will also be responsible for the hand over to the out of hours team at the end of the working day.

KEY DUTIES AND RESPONSIBILITIES
Leading a clinical team to provide proactive and reactive general medical services to the housebound population, including those in care homes, in collaboration with registered practices where appropriate.
Promote and deliver a multi-skilled team response that includes GP Acute Home Visits, holistic assessment, care, pro-active follow up and care planning;
Ensure that the Acute Home Visiting and Community Rapid Response Service that supports and links well to system infrastructure provided at a bigger scale e.g. Acute Hospitals, 999, 111, Ambulance services and social care;
Developing expertise within the community for improving the lives of people living with frailty.
Maximise best care in the patient’s own home in order to reduce the need for hospital or care home admissions.
Provide medical expertise in the management of older people living with frailty in the defined community. To support ACPs and wider MDT members working in the community by:
Providing Senior GP clinical triage for all Acute Home Visit requests to determine the urgency and type of response needed, according to clinical need.
Support other AHPs in the team with
Regular debrief sessions for patients on their caseload.
Formal and informal education for the ACPs during clinical interactions
Where there are difficult clinical risk decisions.
QUALIFICATIONS: -A vocationally trained and accredited GP -Current registration with GMC -On the GP performers list

- Experience and evidence of an interest in care of the Elderly - Understanding of adult safeguarding and Deprivation of liberty procedures - Experience of multidisciplinary working - Experience of senior clinical triage to determine the urgency and type of response needed, according to clinical need
