Salary Band 4 £23,949 to £26,282 per annum
Job summary
This is an exciting opportunity for the ‘right’ person who wants to work supporting patients by coordinating their health and social care services. Based in a GP practice, the care coordinator will be involved in supporting the clinical team to proactively identify and work with patients, including the frail/elderly and those with long-term conditions, to provide proactive, person-centred care planning, helping coordinate care, by bringing together the different specialists whose help that individual might need. This might involve a wide range of services, such as hospital care, community care, social care, housing and the voluntary sector.
Main duties of the job
The Patient Care Coordinator will work closely with the GPs and other primary care professionals within the practice to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their careers, and ensuring that their changing needs are addressed.
About us
The Practice currently has a list size of approximately15,500 patients. The Practice area covers virtually the whole of the City of Derby with two concentrated sub-populations, one around each surgery. In the inner-city terraced housing predominates with council housing on the outskirts particularly around Chaddesden. We have patients with a range of medical issues reflecting health inequalities due to a mix of social and deprivation problems. We also have a wide range of ethnic diversity in our patients.
