JOB SUMMARY 

An exciting opportunity has arisen to join the Practice as a salaried GP due to our popularity and consistently rising list size. 

Long Eaton is a thriving town in between Nottingham and Derby. It provides an exciting mix a patient demographics with a strong sense of local community. 

We are an established training practice with consistently high QOF achievement. We encourage innovation in developing new models of care. You will enjoy support from a large clinical team, and dedicated management colleagues. All Salaried GPs are allocated a Mentor. For those who have partnership aspirations, we offer routes to partnership and have a unique development programme.

PRACTICE SUMMARY 

West Pak Surgery is a recently merged practice with an Outstanding CQC rating at the Long Eaton Health Centre site and a Good rating at Park View Site, we are a secure, successful and forward looking practice. We care for 17,000 patients and also provide in reach migrant health care locally. 

The GP Partners (Dr Jacqui Luff, Dr Duncan Gooch, Dr Ruth Gooch and Dr Rebecca Gladstone) hold key leadership positions in the PCN and the ICS and can provide a wealth of opportunity and experience to other members of the team. Involvement in areas outside of direct clinical practice is encouraged. 

We are part of Erewash Health Partnership and have access to a wide range of networked services including home visiting, care home support team, pharmacists, social prescribers and first contact physios. We offer all core services as well as minor surgery and contraceptive services. 

Education remains an important part of the ethos of the practice, and we train registrars, medical students, non-medical prescribers as well as supporting apprentices from the local community. This culture of continuous improvement and development is core to who we are. Regular clinical meetings across both sites, network learning events and an allocated mentor mean that we create a safe and supportive environment. 


JOB RESPONSIBILITIES 

In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking, and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. 

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. 

Assessing the health care needs of patients with undifferentiated and undiagnosed problems 

Screening patients for disease risk factors and early signs of illness 

Developing care plans for health in consultation with patients and in line with current practice disease management protocols 

Providing counselling and health education 

Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate 

Recording clear and contemporaneous consultation notes to agreed standards 

Collecting data for audit purposes 

Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible) 

Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate 

In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care 

Awareness of and compliance with all relevant practice policies / guidelines, e.g. prescribing, confidentiality, data protection, health and safety 

A commitment to life-long learning and audit to ensure evidence-based best practice 

Contributing to evaluation / audit and clinical standard setting within the organisation 

Contributing to the development of computer-based patient records 

Contributing to the summarising of patient records and read-coding patient data 

Attending training and events organised by the practice, PCN or other agencies, where appropriate 


SALARIED GP DEVELOPMENT PATHWAY 

SALARIED GP (ENTRY LEVEL) 

· Allocated partner mentor 

· 1 hour dedicated mentoring with allocated partner per month plus ad hoc time 

· Structured surgeries 

· Support for fellowships and external interests 

· Opportunity for education and clinical supervision of registrars and medical students 

· £10k per session 

SALARIED GP WITH ENHANCED RESPONSIBILITIES (ENTRY LEVEL ONLY IF RECRUITED INTO THIS ROLE) 

· Allocated partner mentor 

· Lead role for a significant practice area eg Educational Supervisor, Clinical Lead for safeguarding, Caldicott Guardian, Minor Surgery lead etc 

· Protected time for additional responsibility 

· £11k per session 

ASSOCIATE GP PARTNER 

· Work in role of GP partner with senior leadership roles within practice 

· Protected from liability within salaried GP employment structure 

· Low risk way of testing whether partnership is correct 

· £10 k per session plus performance related bonus
