South Hardwick PCN are looking for a Community GP to support our multi-disciplinary team providing healthcare to our housebound patients and patients in care or residential facilities across the PCN footprint.

This is an exciting opportunity to be at the forefront of the delivery of our new Ageing Well model of care - Team Up! Derbyshire. The role transcends the traditional health and social care organisational boundaries. It will involve true MDT working, building relationships with GP practices, Community Rapid Response services and Falls services, to help clinically deliver the service on a day-to-day basis.

This is an evolving service, so you will have the opportunity to contribute to the development of Team Up! and the Home Visiting Service if you wish to.


Main duties of the job

South Hardwick PCN are seeking to recruit a team of Community GPs to work alongside a multi-disciplinary team providing care to housebound patients and patients in care or residential facilities, either in person or remotely.

The aim for the service is to ultimately provide a holistic approach to acute on-the-day/rapid response services, enhanced health in care homes and enhanced proactive care for older people with frailty and patients with multi-faceted health problems.

Key Duties Tasks and Responsibilities

Provide day-to-day clinical leadership for the multi-disciplinary team including debriefing after visits if needed

Home visiting for clinically complex cases e.g., end of life care, diagnostic uncertainty, complex prescribing decisions

Senior clinical triage for the Acute Home Visiting service

Build relationships across the extended team including, but not limited to social care, ambulance, social prescribers, community, and mental health clinicians. 

Opportunity for involvement in service development as the team expands

Involvement in MDT meetings

Provide holistic comprehensive assessment and enhanced advance planning

Maximise best care in the patients own homes to reduce the need for hospital or care home admissions. 

Provide end of life care in line with the patients wishes

Quality improvement activity

Opportunities for teaching and mentorship

MRCGP 

Recognised qualification in Care of the Elderly

Recognised qualification in medical education/clinical supervision

Evidence of leadership development

Experience

Essential

Experience and evidence of an interest in care of the Elderly 

Understanding of adult safeguarding and Deprivation of liberty procedures

Experience of multidisciplinary working

Experience of senior clinical triage for the Acute Home Visiting to determine the urgency and type of response needed, according to clinical need.

Desirable

Knowledge and experience of carrying out Comprehensive Geriatric Assessment

Experience of medical education/clinical supervision

Experience of using SystmOne or another clinical system

Experience of working in a multi-disciplinary team.
