The post holder is a frailty nurse who acts within their professional boundaries, working within a general practice setting within the Primary Care Network (PCN).

The frailty nurse is a key addition to the PCN clinical team and will work under the mentorship of the Clinical Director with daily support from the PCN Complex Care Nurse, Care Co-ordinators, Clinical Pharmacist, Pharmacy Technicians, Home visiting Paramedics, Health Connectors and Practice GP’s and nursing staff, contributing to acute and chronic medical care of all patients within the network.

The post holder will be an integral part of the PCN Complex Care Team.

The primary function of the role of the frailty nurse is to maximise patients’ health, clinically assess and treat patients in a defined group and reduce risks that contribute to ill health. In doing so, they will help to reduce unnecessary admissions to acute services, reducing demand on GP time.

This role will facilitate the delivery of efficient, effective, co-ordinated and timely high-quality care to frailty patients.
