The post holder is a Complex Care lead nurse who acts within their professional boundaries, working within a general practice setting within the Primary Care Network (PCN).

The Lead nurse is a key addition to the PCN clinical team and will work under the mentorship of the Clinical Director with daily support from the PCN Frailty Nurse, Care Co-ordinators, Clinical Pharmacist, Pharmacy Technicians, Home visiting Paramedics, Health Connectors and Practice GP’s and nursing staff, contributing to acute and chronic medical care of all patients within the network.

The post holder will be an integral part of the PCN Complex Care Team.

The primary function of the role of the Complex Care Nurse is to oversee the team daily with contacting patients placed on the hub’s discharge list by any of the 5 practices.

The team of Care Coordinators are responsible for contacting patients who have recently been discharged from hospital to see how they are and if they have any queries or require further support. The patients are added to the Complex Care Hub appointments list by the patient’s respective GP surgeries, from which they are contacted by one of the Care Coordinators.

The CCH team will then coordinate any care needs that are identified and update the patients Emis record. (See Appendix A – Complex Care Hub Activity).

Mendip PCN is a group of 5 practices that work together whilst maintaining their own staff and ethos.

The practices are -

Beckington Family Practice, Grove House Surgery, Shepton Mallet, Mendip Country Practice, Coleford (Host Practice), Oakhill surgery and Park Medical Partnership, Shepton Mallet. The Clinical Director is Dr Rebecca Duffy, GP Partner at Mendip Country Practice
