Community Advanced Nurse Practitioner (Full-time, Part-time, Job share, Flexible working)

ARCH (Alfreton, Ripley, Crich and Heanor) Primary Care Network

ARCH PCN is seeking a Community ANP to help lead our multi-disciplinary and multi-agency team of professionals working across the PCN area including Alfreton, Ripley, Crich and Heanor.

This is an exciting and unique opportunity to be at the forefront of the delivery of our new Ageing Well model of care part of TeamUp! Derbyshire. The role will operate across the traditional health and social care organisational boundaries, including with our GP practice partners, to help clinically lead the service on a day to day basis. The aim for the service is to ultimately provide an holistic approach to our acute home visiting service, enhanced care into care homes and enhanced proactive care for older people with frailty for patients with multifaceted health problems.

Main duties of the job

To support the clinical team delivering this service we are looking for a Community Advanced Nurse Practitioner with a strong interest in working with people with frailty and who aspires to promote a continuous improvement approach to leading and delivering responsive, safe, patient-centred and effective care. The successful candidate will work alongside other clinicians to develop new and responsive clinical pathways for seamless working, shape and champion the introduction of new clinical roles, challenge traditional ways of working and proactively support individuals within the team to develop the necessary skills, competencies and expertise needed to deliver the service.

This post will be hosted by Amber Valley Health- the umbrella organisation for ARCH PCN. The PCN consists of 9 GP practices (89,000 patients) across the PCN area, which operates as 3 neighbourhoods. The PCN is committed to improving the care of patients in the local geographical area and sustaining General Practice.

Job responsibilities

Key Responsibilities and Duties:

The PCN ACP areas of responsibility include:

Providing same day urgent response to housebound patients

Collaborating with a clinical team to ensure care home residents receive co-ordinated medical care. This includes proactive care planning and consistent support from a multidisciplinary team

Participation in multidisciplinary team meetings

Recognition of opportunities for anticipatory care of suitable patients via treatment plans

Working with registered practice to ensure seamless care.

Leading and coordinating across organisations.

Delivering high quality, methodical and well-informed patient diagnoses and care plans.

Developing expertise within the community for improving the lives and care of the elderly.

Promoting the use of supportive, non-statutory services to support self-care and social prescribing agendas.

Maximise best care in the patients care home in order to reduce the need for hospital admissions.

Relationship Management:

Local clinicians, including Geriatricians, ACPs, community Matrons, GPs, members of the local integrated community service, out of hours and urgent care providers, care coordinators and care home workforce.

Local providers of health and social care including the Local authority, GP practices, and local integrated community service

Local hospitals, community mental health and Community health services provider.

Independent Prescribing or study of pharmacology & therapeutics at post-graduate level (Level 7)
