PCN Pharmacist – Dartford Model PCN

Dartford MODEL PCN is a patient centred organisation continuously striving to improve the care of its population. The PCN which is located in Dartford, Kent consists of three practices; The Wellcome Practice, Lowfield Medical Centre and Dr C.J. Shimmins & Partners and serves a patient population of around 30,000.
We are seeking an enthusiastic PCN pharmacist to join our existing PCN pharmacy team to support in improving medicines management and medicines optimisation across the three practices within the PCN. This role is a site based role and the successful candidate may need to travel between the different practices. Please see the job description for further details.
