Fixed Term (Two Years) Advanced Nurse Practitioner

 

An exciting opportunity has arisen to join our friendly and innovate Primary Care Network (PCN) Poole North as we seek an Advanced Nurse Practitioner(s) to help support the acute, on-the-day need of our patients. This post can be either full or part time and you will have a "base practice" of two locations so you can really get to know the teams and the patients rather than working across the whole network.

The ideal prescribing ANP candidate will manage a daily caseload and deal with a wide range of health needs across our modern and spacious practices ensuring the highest standards of care for all registered and temporary patients.

It is a requirement of the role that the successful applicant will have got or will be willing to work towards an Advanced Practitioner Digital badge which can usually be claimed after successful completion of your Masters which you will have undertaken to become an Advance Nurse Practitioner. Main duties of the job

The post-holder will:

  • act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate in a variety of appointment types (face to face, telephone, video.
  • Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
  • Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services.
  • Assesses diagnoses and treat patients attending primary health care services with a range of acute, non-acute and chronic medical conditions.
  • Assess diagnoses and treat patients in surgery who require acute medical attention and refer appropriately in line with local and national guidance.
  • Admit patients to secondary care for acute medical need.
  • Refer patient as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for x-ray in line with policy and appropriateness.
  • Order blood, x-ray and other pathology testing via ICE.
  • Make critical judgements to satisfy the expectations and clinical need of the patient, where appropriate.
  • Prescribes independently in line with the formulary.

 

Poole North Primary Care Network is an innovative, friendly and supportive PCN of four GP practices who work well together in providing services for our 52,000 patients. By providing additional ANP support in our practice locations we are aiming to ensure patients have better access to GP services.
