We are looking for an Advanced Nurse Practitioner / Nurse Practitioner / Paramedic Practitioner to join the team at The Bruton Surgery for 14 hours per week, working on Tuesdays and Wednesdays, on a permanent basis.

Salary is dependent on experience but ranges from between £39,439 - £58,269 per annum, pro rata for Masters educated Advanced Nurse/Paramedic Practitioners with strong proven experience of working in Primary Care.

The post holder should be an experienced nurse or paramedic who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills at an advanced level, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

The ideal candidate would have experience in minor illness/injuries, respiratory and paediatrics, and should be a Non-Medical prescriber.

Main duties of the job

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with highly complex needs

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Support and manage health needs of women presenting for family planning or sexual health consultation

Assess, identify and refer patients presenting with mental health needs in accordance with theNSF for Mental Health

Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses and liaise with GPs to agree the appropriate follow-up treatment and care plans.

Person Specification

Essential Qualifications

Registered first level nurse

MSc or equivalent

Independent/supplementary nurse prescribing qualification

Advanced nursing specialist qualification

Relevant nursing/health degree

Clinical supervision training and experience

Essential Experience

At least 2 years recent primary / community nursing experience

Nurse prescribing

Nurse-led management of minor illness, minor ailments and injuries

Nurse-led triage

Compiling protocols and clinical guidelines
