Job summary
The Care Coordinator for Inequalities and Access will work with the PCN member practices supporting them to identify and address barriers to help improve patient access to the full range of general practice services. Some groups of patients do not currently experience easy access to general practice services and subsequently do not experience the same health outcomes as the rest of the population. We are looking for someone who will champion the needs of those most vulnerable to poor health outcomes who often struggle to access healthcare.
This is a new role across the Primary Care Network supporting practices to implement and deliver NHS England PCN service improvement requirements.
Main duties
The Care Coordinator for Inequalities and Access will work with the PCN central team and the five member practices, supporting them to improve access for hard to reach groups of patients. You will reach out to those communities who sometimes feel voiceless to give them the help and support they need and use assertive outreach skills to engage people who are hard to reach.
You will support patients in preparing for, or in following up, clinical conversations they have with PCN primary care professionals. You will be working with patients to help ensure they have the right support and to signpost to the relevant local voluntary sector organisations that can support them with their non-clinical health needs. To be able to fulfil and enjoy this role, you would benefit from having worked in health or care settings either in a patient/resident-facing or administrative role
Overview of organisation
Taunton Central Primary Care Network is a relatively new organisation, but our five member practices have a strong history of collaborative working to develop the best patient-centred care and services. The PCN has approximately 60,000 patients registered with four practices in Taunton and one practice in the neighbouring village of Bishop's Lydeard. We pride ourselves on our ability and willingness to adopt innovative ways of working that improve patient care and make our PCN a rewarding place to work.
