Job summary

St James Medical Centre is looking to expand the urgent care team. We are looking for an enthusiastic and patient centred practitioner to join our team.

The Advanced Care Practitioner will work alongside the GPs dealing with acute and minor illnesses. This role is expected to develop and evolve over time as the successful post holder gains experience. Further training may be available for the successful candidate.

The successful candidate will be organised, enthusiastic and a pro-active practitioner to our busy multi-disciplinary team.

The successful candidate will demonstrate safe, clinical decision-making and expert care for their patients. The candidate will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing professional support to colleagues as required.

The post holder will be an experienced healthcare professional, who will act within their professional boundaries, they will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of the patient's care.

As an Advanced Care Practitioner, you will deliver a high standard of patient care, using autonomous clinical skills, and will have a broad and in-depth theoretical knowledge base.

The successful candidate will manage a clinical caseload, dealing with presenting patients' needs in a primary care setting.


Main duties of the job

Work collaboratively within the practice team to meet the needs of our patients and supporting the delivery of policy and procedures.

Act within your professional boundaries to provide care of our patients. This includes initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.

Demonstrate safe, clinical decision-making and care.

Demonstrate critical thinking in the clinical decision-making process but with the full support of the wider practice team.

The post holder for this role must have access to car as this role might involve some travelling.


Overview of your organisation

St James is a friendly, high achieving practice based over two sites with a registered list of 18,000 patients. St James is based in the centre of Taunton which is the County town of Somerset.

The post holder will be a part of a supportive team, which includes 7 GP partners, salaried GPs, ACPs, a team of Primary Care Network (PCN) staff, which includes FCP, pharmacists, wellbeing coaches and care coordinators. As well as a supportive management, reception and administration teams.
