We are expanding our Pharmacy Team so an exciting opportunity has arisen to join our established team, you would be working as part of a large team, currently comprising of Clinical Pharmacists and Pharmacy Technicians. We cover a demographically diverse population of more than 63,000 patients. We are looking for someone with an extensive experience in chronic disease management. Ideally, an experience of running a long-term condition clinic is desirable. Holding the CPPE pathway and independent prescribing is desirable, however training and support to complete these in post will be provided.
You will work closely with all GPs, Nurse Practitioners, Paramedic Practitioners, and the wider multi-disciplinary team, which also includes a full nursing team, specialist diabetes and respiratory nurses, social prescribers, dieticians, podiatrists and first contact physiotherapists.
With time and training, the expectation is that the role will develop in line with other activities outlined in the job description. The ideal candidate will therefore be a forward-thinking, enthusiastic individual with initiative, keen to work within a team of like-minded people.
Appropriate training will be provided with support from GP Prescribing Lead and Pharmacy Team Lead as required, experience of the clinical pharmacist role in a primary care setting would be an advantage.

Working Pattern
Up to 37.5 hours to be worked over 4 or 5 days in the week. Whilst there is a degree of flexibility over the pattern, we do require the successful candidate to work Mondays, Thursdays, and Fridays.
Rate of Pay
£49,242 - £56,818 per annum FTE (£25.25 - £29.14 per hour) dependent on experience.
