Base: Derby sites
Hours: Up to 37.5 hours per week
Salary: £43-£48,000 p.a (dependent on experience) and NHS Pension
Job Summary
The post is for a PCN clinical pharmacist as part an ARRS role. The pharmacist will be employed by the practice and will work within, and be supported by, a well established pharmacy team. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews including patients with complex polypharmacy. The post holder will provide support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescribing process, deal with acute prescription requests, and undertake medicines reconciliation for the relevant patients following discharge from hospital.
They will provide expert advice on medicines to patients and the multidisciplinary teams across our practices. Other responsibilities will include the provision of clinical leadership on medicines optimisation, quality improvement and the management of some aspects of the quality and outcomes framework and some enhanced services across all sites. The post holder will facilitate integration with community and hospital pharmacy to improve overall patient care. The role is pivotal to improving the quality of care and operational efficiencies so requires the post holder to demonstrate motivation and passion to deliver excellent services across our practices. The post holder will be supported to develop their role to become a non-medical prescriber as needed. The post holder will be expected to work independently and also as part of our pharmacy team. They must recognise the roles of other colleagues within the organisation and other stakeholders (including patients, senior and peer colleagues, and professionals from other NHS/private organisations e.g. CCGs). They must be able to recognise their personal limitations and refer to more appropriate colleague(s) when necessary.
Primary Duties and Areas of Responsibility
- Long term condition medication reviews
- Care home medication reviews
- Management of common/minor/self-limiting ailments
- Supporting medication queries and information to both practice staff and patients
- Medicines reconciliation following discharge from hospital
- Delivering local enhanced services
- Repeat prescribing
- Clinical audits
- Medication queries form clinicians
- Participate in regular supervision sessions and appraisals with your line manager.
- Action MHRA alerts
- Provide guidance on managing out of stock medication queries
- Attend training and development activities as identified and participate in meetings as required.
- Maintain a good working knowledge of Health and Safety procedures and fire precautions.
- Work flexibly to meet the needs of our patients.
- To undertake any other duties appropriate to the grade and purpose of the job as may be agreed by the post holder or their line manager
