About North Sedgemoor Primary Care Network:
We are an innovative and forward thinking Primary Care Network (PCN) who believe that by all working together we can make a difference to both our staff and patients.

As well as delivering the services set out in the PCN Enhanced Service specification this year we are setting up a 'Same Day Urgent Care' service. We also are looking to expand our proactive care work in the community and continue with the delivery of our Enhanced Healthcare in Care Homes (EHCH) multi-disciplinary team (MDT).

Our PCN is based in North Sedgemoor. Formed of five practices, we provide care for a population approximately 48 000. Our demographic is diverse, meaning there is scope for any PCN employees to develop in a variety of directions beyond routine clinical work. The prospective post holder will find this both challenging and rewarding.

The 5 practices are: Axbridge and Wedmore Medical Practice, Brent Area Medical Centre, Burnham Medical Centre, Cheddar Medical Centre, Highbridge Medical Centre.

We have a PCN Hub, based at the Burnham Health Campus, Brent Road, Burnham on Sea.

The PCN Team consists of Complex Care GP's, Clinical Pharmacists and Pharmacy Technicians, Occupational Health Therapists, Health Coaches, Care Coordinators, a Services Development Lead as well as a Transformation Lead, Digital Lead and Operations Lead.

Our Vacancy:
We are expanding our team of GPs, for both our EHCH complex care team and our developing Same Day Urgent Care Hub (SDUC).

This is an interesting and varied position allowing the successful applicant to have a portfolio style role within one employment.

Part of the week will be working with our multi disciplinary clinical team (Clinical Pharmacists, Pharmacy Techs, OT and Care Coordinators), looking after our more complex and frail patients in care homes, and the community. Our well-established MDT supports the delivery of pro-active care.

The other aspect of the role will include supporting our developing SDUC Hub. We are aiming to offer care for patients who need acute care by forming a multidisciplinary team consisting of GPs, ACPs and paramedics as well as support staff. This will involve seeing patients from all of the 5 PCN practices at a hub.
