This is an exciting opportunity to be at the forefront of the delivery of our new Ageing Well model of care - Team Up Derbyshire.
It will involve true MDT working, building relationships with GP practices, Care Homes, Community Rapid Response services and Falls services, to help clinically deliver the service on a day-to-day basis.
South Hardwick PCN are looking for Advanced Clinical Practitioners and Trainee Advanced Clinical Practitioners to work alongside Community GPs and other members of a multi-disciplinary team to provide healthcare to our housebound and care home patients across the PCN footprint. This is an evolving service, so you will have the opportunity to contribute to the development of Team Up and the Home Visiting Service if you wish to. Trainee ACPs will be supported by the team's Community GPs, Clinical Lead ACP and ACPs, and will also be supported throughout their education and development.
Main duties of the job
South Hardwick PCN are seeking to recruit Advanced Clinical Practitioners and Trainee ACPs to work alongside a multi-disciplinary team providing care to housebound patients and patients in care or residential facilities.
The aim for the service is to ultimately provide a holistic approach to acute on day/rapid response services, enhanced health in care homes and enhanced proactive care for older people with frailty and patients with multi-faceted health problems.

Registered Nurse - registered with RCN/NMC
Paramedic - Registered with HCPC, HCPC recognised Qualification/on Paramedic Practitioner Pathway
MSc in Advanced Clinical Practice (or equivalent)
Evidence of post-registration study at undergraduate level (Level 6)
Independent Prescribing or study of Pharmacology and Therapeutics at post-graduate level (Level 7)
Experience of mentoring and coaching
