Are you good with people, a listener and motivator, flexible and adaptable with an understanding of voluntary and statutory services across Mendip?
We are looking for a compassionate, self-motivated and organised person who thrives on supporting others 1:1 to join an innovative primary care project in Mendip. The person will work in a person centred way to help local patients to manage their health and wellbeing, make lifestyle changes, access health and community services and develop their support network. The role will involve working in doctor’s surgeries, people’s homes and in a range of other health and community settings. Health Connections Mendip works within a multi-disciplinary team ranging from the voluntary sector to palliative care through to secondary care.
