Torquay Care Home Visiting Service GP
We are looking for enthusiastic GPs with excellent communication skills and an interest in elderly care to join the Torquay Care Home Visiting Service (CHVS) for 1-2 days a week, one of which must be a Friday. We are an innovative service providing dedicated GP cover to 12 care homes in Torquay. We provide acute visits to unwell patients with the aim of starting treatment earlier and reducing hospital admissions. We also do proactive advance care planning, reviews for ongoing conditions and liaison with relatives. Our aim is to improve outcomes for patients while supporting GP practices by reducing their workload pressures.
You will be working within the established pathways of the CHVS and with full access to patients’ medical records. We have regular clinical meetings and work closely with the equivalent service in Brixham and Paignton so there are lots of opportunities for shared learning. This would be an ideal role for any doctor looking to explore different ways of working as part of a portfolio career.
