Patient Surveys

Completed in Three Easy Steps

Step 1

Log in or Register your new account.

Visit the surveys page and create a Patient Survey, then print out 50 copies of the survey.

This help article takes you through the survey set up process.

Step 2

Ask your Receptionist to hand out the surveys with a pen to patients.

Once completed, get patients to put the surveys in a collection box by reception or hand them in.

Step 3

When you have at least 34 completed surveys post them to us, or scan and upload them to save time and postage costs.

We'll take care of the rest and within a few days you will receive an email with your completed report.

Important: If you are posting your surveys please don't staple, paperclip, fold or put the responses in an envelope. We scan the responses before entry and removing staples etc can delay entry.

Is the Patient Survey fit for revalidation?

Yes, the survey has been developed by Wessex LMCs. Wessex LMCs have followed the guidance laid down by the GMC.

How much does the Patient Survey cost?

Just £28

Do you have a refund policy?

Refunds will be offered until the point that we receive the patient questionnaires after which no refunds will be given. If you would like a refund please contact support and we will arrange for a refund.

Do I have to be a member of Wessex LMCs?

No. Any health professional can register and use the site. Doctors who do not relate to the GMC (i.e. outside of the UK) should check with the regulatory body that they relate to. 

Is the Patient Survey only for Doctors?

No, there is also a patient survey tailored for Nurses and Doctors who just perform occupational health medicals.

Who are Wessex LMCs?

Wessex Local Medical Committees are the statutory body that represents GPs and practices throughout Dorset, Hampshire, the Isle of Wight, Wiltshire, Jersey or Guernsey. For more information visit the Wessex LMCs website.

So you will be seeing our Patient responses?

Yes we will but we take your confidentiality extremely seriously and will not be divulging any of your data or the responses to anyone.

How is our data held?

It is held securely and in accordance with the Data Protection Act 1998.

Will we get our paper responses back?

We do not return the paper copies. We keep them for 6 months before securely shredding them.

Shall I send the paper responses by recorded delivery?

This means the delivery can be tracked though our delivery rates are very good - we have only had a 2 lost in the post so far out of over 1500 surveys. We do recommend taking photocopies prior to sending them just in case they are lost in transit.

Remember - you can save on time and postage costs by scanning and uploading your completed surveys to your FourteenFish account.
