This mini cex is regarding the history and examination of a gentleman in his 50s who presented with a 2.5 week occipital and unilateral frontal headache. He had some red flags for raised ICP including exacerbation of pain when coughing and bending forwards. He had a CT head that was NAD and CT intracranial venogram that showed no evidence of venous sinus thrombosis. He had temporal pain but no jaw clarification or scalp tenderness. ESR was normal.

The gentleman was given stat sumatriptan and metoclopramide and IVI. He was unable to stay to enable us to find out if these were effective. If they were effective it was likely migraine. If they were ineffective he was advised to commence amitriptyline (it had worked for him in the past) and we gave a boots prescription for that. We discharged him with patient initiated follow up and the advice to call SDEC if symptoms did not resolve in which case we will complete a neurology referral.

The capabilities I believe were demonstrated in this case were:

1. Data gathering and interpretation. Specifically: “Chooses examinations and targets investigations appropriately and efficiently” (I attempted Fundoscopy and did a full neuro exam, as well as cardio exam given his incidental ecg findings) and “Gathers information systematically using questions appropriately targeted to the problem.”
2. Clinical examination and procedural skills. Specifically 	“Conducts examinations targeted to the patient's problems.”
3. Decision-making and Diagnosis. Specifically “Makes diagnoses in a structured way using a problem-solving method. Adopts appropriate decision-making principles based on a shared understanding. Uses best available, current, valid, and relevant evidence.”
