About Us • Outstanding CQC Rating: Consistently rated Outstanding by CQC • GP patient Survey 2024- 96% describe their overall experience as good https://gp-patient.co.uk/patientexperiences?practicecode=L83116 • 5,400 Patients: A manageable list size enabling continuity and high-quality personalised care. • Same-Day Access: We aim to see many patients on the same day, so we are already compliant with all the requirements of Modern Access to General practice. Our patients have embraced this way of working since 2020, there are no long waits or patients dissatisfied with access • Embracing Technology: We use SystmOne, Ardens, and AccuRx for efficient clinical care. We also use ambient AI scribing to assist in consultations where appropriate. • Innovative Triage: Our systems support rapid, safe, and streamlined access. • Collaborative Team: o 2 GP Partners o 3 Salaried GPs o Full time Clinical Pharmacist o 2 experienced practice nurses with respiratory and diabetes special interests, Nursing Associate, and HCA o On site dispensary for majority of patients (dispensing 7000 items per month) o Friendly and efficient administrative and management team o PCN ARRS team including pharmacy technicians, visiting paramedic, social prescribers, and HeadsUp mental health team for children • Training Practice: We host University of Exeter Medical School students, and nursing and pharmacy trainees. • Research Active: NIHR SW Peninsula Regional RDN RSI level 1 Practice • Committed to learning: Quarterly protected learning afternoons, we embrace learning, development and quality improvement work • Low home visits: we are supported by a PCN visiting paramedic with expertise in frailty, and work closely with an exceptional Hospiscare Team, including their own paramedic and ANP prescribers • Pooled list: we work with a pooled list so that we can combine rapid access where needed, with continuity from preferred GPs for more complex patients. List holding is for admin purposes only, and we have efficient systems to reduce the burden of GP admin and prescription workload. We are always looking for ways to improve our systems.
