On behalf of the general practices of Derby City, Derbyshire Healthcare NHS Foundation Trust is looking to employ a team of Primary Care Mental Health Workers and Occupational Therapists.

This is to provide a supportive role for patients experiencing mental health problems; conducting high quality assessments of people’s mental health needs and using this information to inform formulation, diagnosis and care planning.

Your team will be based with Community Action Derby, from where you will deliver remote consultations as well as visiting your individual Primary Care Networks Practices.

Main duties of the job

The Derby City Primary Care Networks First Contact Mental Health Service is being developed to build the capacity and capability of Derby City General Practice to better manage patients presenting with mental health problems in primary care.

The service provides appointments for Derby City General practice registered patients. Patients are booked directly into appointments by practice reception staff.

You will consult (remotely at first by telephone) patients centrally for all Derby City practices.

As a member of the growing Derby City First Contact Mental Health team, you will provide first contact mental health consultations to patients registered with Derby City General practices.

You will be employed by Derbyshire Healthcare NHS Foundation Trust (DHcFT) in a jointly funded post with Derby City Primary Care Networks.

The role is funded 50:50 through the PCN Additional Roles Reimbursement Scheme and DHcFT.   Although DHcFT fund the roles and provide human resource management and supervision, the post is entirely within general practice with no additional DHcFT duties.
