This role is as diverse as it is challenging, and you will be working as part of a team providing support to the duty doctors within Belper PCN and our advanced home visiting service for the frail and elderly, housebound patients. You will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and management of care through face to face and telephone consultations. You will need to be able to provide treatment to the patient at home, within the Care Home and at one of our practices when required.

We would require you to have good communications skills, an ability to prioritise and manage your own workload and being motivated by delivering exceptional patient care.

The successful candidate will be supported by clinical leads and the extended PCN team.

Main duties of the job

As a General Practice Paramedic, you will work as part of a clinical team, with GP’s, Advanced Nurse Practitioners, First Contact Physiotherapists, Care Coordinators Practice Nurses and Healthcare Assistants. A Paramedic would be involved with diagnosing and prescribing treatments and medication for acute illnesses whist working within defined procedures and protocols as laid down by the primary care network.

You will demonstrate safe clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

The post holder will demonstrate critical thinking in the clinical decision-making process, with the ability to prioritise and triage the needs of the patients, accordingly, instigating appropriate investigations or referrals to colleagues and other care providers.

You will work collaboratively as part of the general practice multidisciplinary team to meet the needs of patients, supporting the delivery of policy and procedures. The roles are both varied and diverse with clinical support and mentorship provided to allow the successful recruit to flourish.

The role will include working as part of the duty team and playing a vital part in performing urgent assessments whether this be remotely, in practice or in a patient's home.

Job Description

Key duties and responsibilities

  1. assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the PCN team
  2. advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.
  3. be able to:

perform specialist health checks and reviews

perform and interpret ECGs; alongside other results as appropriate

perform investigatory procedures as required

undertake the collection of pathological specimens including intravenous blood samples, swabs, etc.

perform investigatory procedures needed by patients and those requested by GPs and the wider PCN

  1. support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing)
  2. provide a service model to urgent and same day home visits for the PCN and undertake clinical audits
  3. communicate at all levels across PCNs and other organisations, ensuring effective, patient-centred service
  4. communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required
  5. collaborate with other members of the PCN including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care
  6. maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice
  7. work with patients in order to support compliance with and adherence to prescribed treatments
  8. provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
  9. assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs
  10. clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  11. prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  12. diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
  13. support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.

Quality Requirements

  1. produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
  2. be aware of data protection (GDPR) and confidentiality issues particularly within a PCN
  3. use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information

  • Experience in managing the care of patients presenting with undifferentiated undiagnosed conditions and/or a designated caseload, using theoretical and practical experience, completing examinations and assessments, care planning, implementing interventions, referring appropriately, and evaluating outcomes aligned to care plans.
  • Ability to write comprehensive, accurate clinical notes, implement and evaluate care plans.Understand own scope of practice, the context of continual learning and the need to develop constantly to ensure safe, competent and confident practice.
  • Ensure that clinical practice is safe and effective and remains within the boundaries of competence, and to acknowledge own limitations.
  • Travel between the different GP Practices within the PCN and home visits will be necessary, so a car driver would required

Desirable

  • Working towards or willing to work towards first contact practitioner (FCP) or advanced clinical practitioner (ACP) status.
  • College of Paramedics Diploma in Primary and Urgent Care (Dip.PUC) exam pass.
  • Mentorship or supervisory skills training.
  • Ability to undertake surgery assessments, procedure and post-operative care
  • Awareness of data protection (GDPR) and confidentiality issues particularly within a healthcare setting.
  • Cognitive behavioural and motivational interviewing approaches / skills.
  • Working knowledge of GP practice software and prescribing data monitoring systems
  • Prescribe, issue and review medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways. This will be done in accordance with evidence-based practice and national and practice protocols, and within scope of practice
