The Humbleyard Practice is looking for an ambitious Nurse or Paramedic or already qualified ANP/ACP to join the Same-Day Outreach Practitioner team. We are based in South Norfolk and care for more than 21,500 patients in the Hethersett, Cringleford and Mulbarton.

The ideal candidate will be a Registered Nurse, Paramedic or Advanced Nurse/ Clinical Practitioner with experience and competencies in triage, assessment and management of patients presenting with acute illness/ injury. The role will include telephone and online triage alongside running clinics and visiting the homes of patients that require same-day assessment.

Job role:
SAME-DAY OUTREACH PRACTITIONER full or part time considered
Essential skills:
EITHER: Registered Nurse with current NMC registration or paramedic with current HCPC registration
OR: Qualified Advanced Nurse/ Clinical Practitioner
Proven experience of triage and assessment, able to prioritise health complaints and intervene appropriately to assist the patient in complex, urgent or emergency situations, including the initiation of effective emergency care
Driving licence and ability to use own car to meet visiting requirements
Desirable knowledge and skills:
Prescribing Qualification
Enhanced Clinical Assessment Masters module/ Minor Illness or Injury qualification
Able to initiate, direct and co-ordinate programmes of care working autonomously and collaboratively, recognising when referrals are required
Able to relay clinical information and maintain accurate, comprehensive consultation records
Organised, friendly, approachable and self -motivated
Adaptable and comfortable dealing with changing priorities and a team player
Work within level of competence
Able to work in a changing environment and implement change when applicable to provide high quality health outcomes
Clinical Responsibilities:
Assess, plan, provide, implement, and evaluate care requirements to meet the care needs of individuals in the practice population, including initiation of emergency care.
Identify and manage as appropriate, treatment plans for patients both at risk of developing a long-term condition and diagnosed chronic disease
Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self-care
Adhere to practice policies and procedures
Agree own actions are consistent with agreed standards of care, following practice / national guidelines
Assess own performance and take accountability for own actions either directly or under supervision
