Due to continued growth we are looking for an experienced Advanced Clinical Practitioner to join our practice.
We are a friendly Practice with extremely good patient satisfaction levels and a "Good" CQC rating across all areas.
Being a forward thinking, organised and structured Practice we have a dedicated holistic approach to providing high quality patient care. We operate from a purpose built surgery on the outskirts of Halifax town centre, which offers a blend of rural and suburban life. There are a number of excellent schools in the area.
We are offering a flexible (up to 8 sessions) salaried position. 6 weeks holiday & 1 week study leave with an attractive salary dependent on experience.

Main duties of the job
The post holder will provide care for the presenting patient from initial history taking, clinical assessment and diagnostic skills, for patients within the general practice. The successful applicant will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
Overview of your organisation
A 5 Partner Practice, serving approximately 8,500 patients. Our friendly team also includes 2 salaried GPs, 2 Practice Nurses, 1 Nursing Associate and 1 HCA and a wonderfully helpful and experienced Administrative Team as well as innovative management.
We strongly believe in the importance of a safe and supportive working environment.
We meet informally on a daily basis for a Clinicians meeting to discuss cases. We have a more formal meeting structure in place to ensure complex and vulnerable patients are discussed in protected MDT time.

Through our active and supportive PCN we have access to Clinical Pharmacists, First Contact Physios, IAPT assessments, Social Prescribing Link Workers and Care Coordinators. Plans are in place to extend this workforce in line with the Network DES.
GENERAL AIM: To facilitate the delivery of, and deliver directly where appropriate, quality primary care services to patients; support the doctors in their daily work, co-operate with other members of the primary care team in their specific duties and maintain good communications always.
JOB RESPONSIBILITIES: All clinical work to be carried out within practice protocols, NSFs, NICE Guidelines and GMS Contract, being clinically responsible directly to the GPs on individual patient matters.
In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone/video/online consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
Developing care plans for health in consultation with patients and in line with current practice disease management protocols
Providing counselling and health education
Referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards
Collecting data for audit purposes
Compiling and issuing computer-generated acute and repeat prescriptions
Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with an Advanced Clinical Practitioner working within Primary care.
