Job summary

This is an exciting opportunity to join a dynamic and well developed workforce in a mature Primary Care Network in the Pudsey / Bramley area. We have one dietitian in post already who has a focus on frailty and we would like to recruit another dietitian with a specialty in diabetes and weight management.We are looking for a highly skilled, experienced practitioner to lead the diabetes and weight management service including clinical supervision of our PCN weight management Health and Wellbeing Coach.PCN dietitians have an opportunity to address health inequalities across large numbers, making a significant impact on the wellbeing of the patients they treat both curatively and preventatively. WLPCN benefits from very robust clinical teams already in place such as pharmacy, occupational therapy, social prescribers, HCA support workers and more which means that our patients can now access all of these services via their own GP surgery. WLPCN also embraces in-house multi-disciplinary team working which is a huge benefit to our patient population

First Contact Practitioner dietitians now need to complete or have completed the Health Education England Roadmap to Practice for primary care. It would be advantageous if you have already completed this although we will support the right candidate to enrol with an educational establishment in order to complete whilst working.

Main duties of the job

Educate patients with diet-related disorders on how they can improve their health and prevent disease by adopting healthier eating / drinking habits; provide dietary support to patients of all ages (from early-life to end-of-life) in a variety of settings. Make recommendations to PCN staff regarding changes to medications for the nutritional management of patients, based on interpretation of biochemical, physiological, and dietary requirements; implement all aspects of effective clinical governance for own practice, including undertaking regular audit and evaluation, supervision and training; have an awareness of your own clinical competencies and ensure that you practice within this and seek support as required.
