Job summary

We are ideally seeking to fill this role with an Advanced Nurse / Clinical Practitioner or an experienced independently prescribing nurse

At West Leeds Primary Care Network we have an experienced and well developed Frailty Team who fulfil the home visiting function for both our elderly population and anyone else who needs an acute on the day home visit. In order to boost capacity within this busy team, we would like to recruit a nurse or advanced nurse practitioner to help with the daily acute home visits. We can be flexible to a degree for the right candidate with regards to how many hours - either part time or full time. This role would suit an NMC registered nurse or an ANP with well rounded healthcare experience who would have the confidence to tackle acute issues such as chest infections, urinary tract infections and wound care. Since the main clinicians in the frailty team are Advanced Practitioners, you would have instant access to high level advice and guidance as you work so this is not an isolated role. Main duties of the job

Key Responsibilities

You will organise your day efficiently, prioritising your own workload as appropriate

You will visit acutely unwell patients in their homes, assessing their needs and treating them accordingly 

You will work as part of a multi-disciplinary team in a patient facing role to assess and work with patients using expert knowledge

You will work with the wider PCN teams as appropriate where an onward referral is in the patients best interest

You will maintain accurate clinical records of all patient consultations and related work.

You will take a keen interest in upskilling and learning, embracing your professional development at all times

The successful recruit will be expected to work across all practices in the PCN, which means they will be working in the community across Pudsey, Bramley, Farsley, Stanningley and Calverley.
