Primary Care Advanced Pharmacist

37.5 hrs per week – Closing Date: 28 April 2019

Taff Vale is a long established teaching Practice covering the Pontypridd and surrounding areas. Our list size is 16,500 patients with our main branch situated just 15 minutes north of Cardiff in Rhydyfelin.

As a practice we are committed to future development and are looking to appoint an enthusiastic and committed prescribing pharmacist to join our highly motivated and growing team .

The successful applicant would be working as part of an extensive team and would be key to developing pharmacist led services within the practice as well as the development of future services, protocols, staff development and education.

Key Role and Responsibilities include:

Plan, organise and undertake pharmacist led clinics such as patient medication reviews , Warfarin clinics, NOAC management etc

Supporting and progressing the development of pharmaceutical services, protocols and education within the practice.

Act as an independent prescriber

Liaise with external colleagues including community pharmacists, secondary care colleagues, local nursing homes and cluster colleagues.

Support the practice/cluster development plans

Providing medicines, therapeutic and pharmaceutical expertise to the GP partners and wider team as needed, producing relevant reports and audits within the practice.

Provide line management support and education to practice prescribing clerks.

Educating other members of the primary healthcare team in specific therapeutic areas and the reasons behind why changes should be instigated.

Communicating and ensuring that all health care professionals within the practice are aware of any medical alerts, national public health circulars, recalls and CSM warnings.

Analysing patient prescribing data and advising on the development of appropriate treatment protocols and care pathways.
